Fill out the forms below. All fields on the form has to be met prior to acceptance. Add the registration fee to your cart and check out. You will receive a conformation email. Palm Beach Rams Football & Cheerleading
Consideration of my child being permitted to participate. I hereby release, waive, indemnify and hold harmless Palm Beach Rams Football and Cheerleading League, volunteers, agents and sponsors from all liability from all injury, loss damages/and or claim of damages to the persons or property, of my child during his or her participation in the Palm Beach Rams Football and Cheerleading League. I hereby grant permission for Palm Beach Rams, and or its agents to photograph and or video tape the above-named registrant during his/her/our participation in activities or use of recreational facilities. I further authorize the use of all photos and/or videos for your child as a to be false/incorrect, may cause your child a denial of participation in our Football/Cheerleading League.
